Transitional Foster HomeThis service, formerly Therapeutic Foster Care (TFC), is an intensive short-term community based treatment service provided to youth in a family setting for youth between the ages of 3-21 with emotional and behavioral challenges. These youth may require a therapeutic, out-of-home setting with trained, licensed and passionate individuals committed to serving and transitioning youth back to their natural caregivers.

Individualized Care and Team Work

Upon intake HBH completes an assessment specifically tailored to address the needs of each youth. An individualized service plan is created by the youth’s team and becomes clinical component of the overall coordinated service plan shared with community partners and evaluated for progress monthly.
